We are a collaboration of people with years of experience in the medical field.  Your first stop for wellness.  Our goal is to provide patient centered individualized health care that follows the most recent guidelines.  Let us be your care for life. 


Please download and fill out the new patient forms and return them to the office.  

Our office staff will then call you to set up your first appointment.

Healow app

This app lets you access you patient portal any time on you phone.  You can request appointments and fill out questionnaires ahead of time online.  You can also do a televisit from your phone.   Messages from your provider can show up in your notifications and you can message them back instantly.

Virtual Visits/ televisits

We can see you in the comfort of your home for follow up or same day visits.  When you call to schedule your appointment you may be offered a televisit.  There are many ways you can connect to the televisit.  If you have concerns please ask out staff for more information or watch the video link below.

Debbie Zinger RN is offering chronic care management and care management to our patients.   If you need any help with medical resources, or help with and social issues that impair your quality of life please call Debbie.

Our goal is for good health: preventitive care is a key to this.   We would like to see every patient yearly and at times an appointment or two in between.  This will help you keep on top of your health care goals.   If you need a refill you may also need an appointment. Best practice is to have a future appointment scheduled every year.

At this time,  we are not accepting refill requets from pharmacies.   If you need a refill please call the office or use your portal to request a refill.  If you are due for a refill you may also need an appointment.  The goal is to keep up to date on your health care.
