MASKS ARE STILL REQUIRED

For the safety of all of our patients, even if you are vaccinated or have had COVID, masks are still required in the office.  

A medical mask is required or a KN95 mask.

Cloth masks or face coverings are not adequate at this time.

Care Management

We continue to grow and provide you with options to stay healthy.

She is offering chronic care management and care management to our patients. 

Telehealth Visits/ Patient Portal / Healow app

Patient portal

The patient portal gives you access to your chart 24/7.  You can request refills, email your provider, update your medical history and ask non urgent questions. The portal also supports health trackers like fit bits and others, so you can upload your health data right into your chart for you and your provider to review. 

If you don't want another password you can use your smart phone to login also.

Healow app

This app lets you access you patient portal any time on you phone.  You can request appointments and fill out questionnaires ahead of time online.  You can also do a televisit from your phone.   Messages from your provider can show up in your notifications and you can message them back instantly.

Virtual Visits/ televisits

We can see you in the comfort of your home for follow up or same day visits.  When you call to schedule your appointment you may be offered a televisit.  There are many ways you can connect to the televisit.  If you have concerns please ask out staff for more information or watch the video link below.

Office policy

Only Patients with appointments will be allowed into the office.  

No visitors or family at this time.


  • Instead of entering the building from the parking lot we are going to have you enter on the street side of the building (closest to M-22). Then exit the building towards the parking lot (bayside).   
  • Our staff will ask you questions prior to allowing you entrance for your appointment.
  • You will be asked to reschedule or stay in your car if you have,
    • Experienced any symptoms, including  
      • Fever above 100.4 degrees, chills, repeated shaking from chills,
      • Cough
      • Shortness of breath or difficulty breathing, 
      • Sore throat, muscle pain, headache, new loss of taste or smell
      • flu-like symptoms or diarrhea, within the past 24 hours
      • Other COVID-19 risk factors (traveled internationally or domestically outside of Michigan, exposure to persons with confirmed COVID-19 diagnosis within the past 14 days, or exposure to someone who has traveled within the last 14 days) 
      • traveled or had visitor from outside their home, within the last 2 weeks. 
  • Maintain 6 foot social distance from others and our staff, unless directed during your exam.  
  • At the mask station, sanitize your hands and put a face mask if you don't have one. Then sanitize your hands again. 
  • Wear the mask at all times.  Only removed if asked by the provider for your exam. 
  • Do not wear your mask around your neck or pull it under your chin or nose at any time.  The mask must cover your nose and mouth at all times.  
  • You may bring your own face mask from home as long as it is a clean medical mask. 
  • When leaving the facility, please follow the one way signs and check out. Then sanitize your hands.

Board certified in Family Practice.   Graduated from Michigan State University College of Osteopathy Medicine with internship and residency in family medicine at Munson Medical Center in 2005.  After residency, practiced in Sault Ste. Marie at the Sault Tribe of Chippewa Indians Medical Center then Brimley Indian Health Clinic as Medical 

Board certified in Family Practice.   Graduated from Michigan State University College of Osteopathy Medicine with internship and residency in family medicine at Munson Medical Center in 2005.  After residency, practiced in Sault Ste. Marie at the Sault Tribe of Chippewa Indians Medical Center then Brimley Indian Health Clinic as Medical Director.  Returning  to Bingham township in 2011, The move back to allowed a better balance of work and home.  Opened Leelanau Family Practice in 2018.  Married to Scott and has two children, Michael and Kathryn who attend Suttons Bay schools.
