For your safety, everyone entering our facility must wear a face mask.

 Only Patients with appointments will be allowed into the office.  No visitors or family at this time.

  • Stay in your car and call the office to inform us you have arrived.
  • Our staff will ask you questions prior to allowing you entrance for your appointment.
  • No person are allowed to enter facility if they have: 
    • Experienced any symptoms, including  
      • Fever above 100.4 degrees, chills, repeated shaking from chills,
      • Cough
      • Shortness of breath or difficulty breathing, 
      • Sore throat, muscle pain, headache, new loss of taste or smell
      • flu-like symptoms or diarrhea, within the past 24 hours
      • Other COVID-19 risk factors (traveled internationally or domestically outside of Michigan, exposure to persons with confirmed COVID-19 diagnosis within the past 14 days, etc.). 
  • Maintain 6 foot social distance from others and our staff, unless directed during your exam.  
  • At the mask station, sanitize your hands and put on your face mask. Then sanitize your hands again. 
  • Wear the mask at all times while in a waiting area or when in your room. Only removed if asked by the provider for your exam. 
  • Do not wear your mask around your neck or pull it under your chin or nose at any time.  
  • You may bring your own face mask from home as long as it is clean and not ripped. 
  •  When leaving the facility, remove your mask at the exit, put the mask in the laundry bag and sanitize your hands.  

For the health and safety of patients, visitors and our health care staff, we reserve the right to deny visitation; reschedule patient appointments; or make alternative treatment arrangements at any time. We appreciate your patience and cooperation during this time.

We are available for your healthcare needs during the COVID-19 crisis.  Staying healthy is very important.  Call the office and we will schedule you for a virtual visit.  You will get to be seen through the HEALOW app. , the patient portal or just a link sent to your phone.  Keep in touch, but STAY HOME.

Chronic Care Management

For patients with Medicare, we are starting a program to keep in touch with you by phone.  You may call our office to enroll if you have not already heard from us.

Patient portal

 The patient portal gives you access to your chart 24/7.  You can request refills, email your provider, update your medical history and ask non urgent questions. The portal also supports health trackers like fit bits and others, so you can upload your health data right into your chart for you and your provider to review. 

If you don't want another password you can use your smart phone to login also.

Healow app

You can request appointments and fill out questionnaires ahead of time online.  

Carry your complete medical record with you on the HEALOW app. 


To set up a televisit you can request an appointment in the patient portal or just call the office.

Emergency warning signs

If you develop EMERGENCY (SEVERE) warning signs for COVID-19 get medical attention immediately.   

If you are severely ill and think you may require hospitalization:  

  • Go to the Emergency Room


EMERGENCY warning signs include*:

  • Difficulty breathing or shortness of breath
  • Persistent pain or pressure in the chest
  • New confusion or inability to arouse/awaken person
  • Bluish lips or face

Board certified in Family Practice.   Graduated from Michigan State University College of Osteopathy Medicine with internship and residency in family medicine at Munson Medical Center in 2005.  After residency, practiced in Sault Ste. Marie at the Sault Tribe of Chippewa Indians Medical Center then Brimley Indian Health Clinic as Medical Director.  Returning  to Bingham township in 2011, The move back to allowed a better balance of work and home.  Opened Leelanau Family Practice in 2018.  Married to Scott and has two children, Michael and Kathryn who attend Suttons Bay schools.

Board certified Family Nurse Practitioner, Masters of Science in Nursing from Michigan State University. Jennifer has been working with Dr. Paquette for 4 years and has specialized

in helping patients optimize their health throughout their lifespan and in working with patients with issues surrounding mental health.  She has also focused on encouraging healthy lifestyle changes in patients of all ages through nutrition, exercise, supplements and stress reduction.  Jennifer loves living adjacent to the Sleeping Bear National Lakeshore with her family and three dogs and is thrilled to be part of Leelanau Family Care.
